Care transition involves moving care recipients between sectors, locations, or care providers to receive care and services or undergo tests and treatments. Care transition can be a stressful time for care recipients and a vulnerable time for workers. Fear, uncertainty, and loss of control can quickly lead care recipients to act violently towards healthcare workers and cause physical and/or psychological injuries. Violence during care transition can be prevented by planning, proactively sharing information, and having the right resources.

The webinar is for workplace parties (i.e., employers, supervisors, workers, joint health and safety committee members, and health and safety representatives, safety professionals, unions) from different organizations and sectors including hospitals, long-term care facilities, home and community care agencies, Local Health Integration Networks (LHINs), emergency medical services (EMS), police and corrections, and courts who develop, put in place, or evaluate their organization’s care transition policy, measures, or procedures.
